d&rsquo;information partag\u00e9) a \u00e9t\u00e9 cr\u00e9\u00e9 \u00e0 la Maison de Sant\u00e9. Cette secr\u00e9taire travaille sur la pr\u00e9vention, dans le but d\u2019am\u00e9liorer le suivi des patients (d\u00e9pistages, vaccinations et inclusion dans les protocoles pluriprofessionnels).<\/p><p>Les principales missions de ce poste consistent \u00e0 :<\/p><ul><li>structurer l&rsquo;information dans les dossiers patients pour que ceux-ci soient complets et que les donn\u00e9es puissent \u00eatre utilis\u00e9es dans un objectif de pr\u00e9vention. Il s&rsquo;agit notamment de rep\u00e9rer les patients qui ne seraient pas \u00e0 jour de leurs vaccinations, de leurs d\u00e9pistages ou de leur suivi m\u00e9dical.<\/li><li>aller vers les patients qui ne r\u00e9alisent pas leur suivi ou ne participent pas aux actions de pr\u00e9vention pour leur proposer les rendez-vous de consultation n\u00e9cessaires (pour les d\u00e9pistages ou pour des consultations de second recours par exemple), de l&rsquo;ETP (\u00e9ducation th\u00e9rapeutique du patient) cibl\u00e9e et\/ou des r\u00e9unions de pr\u00e9vention adapt\u00e9es.<\/li><\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b016214 elementor-widget elementor-widget-text-editor\" data-id=\"b016214\"
